COLUMBIA CREDIT UNION ELECTRONIC STATEMENT
DISCLOSURE AGREEMENT

Description of Service and Consent
By clicking "Continue," you consent and agree to permit Columbia Credit Union ("CCU") to provide monthly e-statements, legal disclosures and notices ("Documents") to you in electronic form, instead of providing such in written form ("Electronic Document Delivery"). CCU will use your online banking user name and password to authenticate your acceptance.

You consent to receive E-Statements and the following related documents electronically.

E-Statements and Billing Error Notice & Credit Union Annual Privacy Notices. The E-Statement delivery will include periodic account statements, future disclosures required by law, other notices, newsletters, special offers and promotional materials electronically. E-Statements will contain the same information as the paper statements you have been receiving, which may include:

Once you consent to the above E-Statements, we will provide the E-Statement service and discontinue paper statements for all of your deposit and loan accounts currently existing or any you open in the future.

If you would like a paper copy of a monthly statement, you may complete the secure request form from within Personal Online Banking, contact us at (360) 891-4000, outside of Vancouver at (800) 699-4009, or stop by one of our branch locations. See the CCU Fee Schedule for any fees that may apply.

System Requirements
To access your statements electronically, your computer system must meet the following minimum requirements:

You are solely responsible for the cost and maintenance of such equipment and access to the Internet.

Email Communications
You agree to designate an email address for e-statement notification and to notify CCU if this email changes. When your new statement is available online, CCU will send a message to the email address you designate and deem it sent and received at that time. If in the event you do not receive such an email notification, it is your responsibility to periodically log on to your Personal Online Banking Account and check the delivery of new Electronic Documents under E-Statements. Additionally, please check any email filtering software you may be using to ensure that it does not screen out email notifications from CCU. You are solely responsible for accessing your email for Documents. CCU is not obligated to monitor whether you are accessing your Personal Online Banking account to retrieve statements.

If your email address changes, you may call our Member Service Call Center at (360) 891-4000, outside of Vancouver at (800) 699-4009 or click "My Settings" within Personal Online Banking and select "Edit" next to primary email. If your email address becomes invalid, you will not receive notification that your e-statement is available, nor will you receive a paper statement to your mailing address on record.

Fees or Restrictions
There are no account fees or service restrictions for choosing or not choosing to consent for E-Statements or other electronic disclosures.

Your Right to Terminate
Your cancellation of electronic disclosure delivery will apply to all E-Statements for your accounts and other electronic disclosure listed above. You may not cancel E-Statements for one account and retain E-Statements for another account. Your cancellation will become effective after we have received it and had a reasonable opportunity to act on it. Paper copies of past statements are subject to the standard duplicate statement fee as provided on your Fee Schedule.

You may withdraw your consent to receive e-statements at any time by providing us with your instructions either by contacting us at (360) 891-4000, outside of Vancouver at (800) 699-4009, stopping by one of our branch locations, or providing written notice by mail.

Our Right to Terminate
You agree that we may terminate e-statements and revert to printed, mailed statements for any reason at any time. Your ability to access and view e-statements is dependent upon being an active user of Personal Online Banking. If you fail to use your Online Banking Password for 180 days, your online banking access will be eliminated. We will send emailed notices that your statements are available for viewing every month. Should you decide to view your e-statements, please contact us to reactive your personal online banking account.

Warranties/Limitation on Liability
CCU cannot always foresee or anticipate every technical or other difficulty or problem that you and/or CCU may encounter or experience with this agreement or the e-statements. Thus CCU makes no warranty that Personal Online Banking/E-Statements will meet your specific requirements or function in an uninterrupted or error free manner. If you encounter problems using e-statements or accessing the e-newsletter, please contact CCU. You represent and warrant that you have the computer equipment and access capabilities to receive documents electronically under this agreement.

Other Agreements
You understand and agree that your use of CCU's Electronic Document Delivery is subject to the terms of any other member agreement(s) you may have with CCU.
